CHECK LIST FOR A MEDICAL TREATMENT VISA
Form 48ME must be fully completed.
Letter from doctor/hospital confirming acceptance of applicant for treatment. Letter must contain information on duration, cost of treatment and that no Australian citizen or Australian permanent resident would be disadvantaged in obtaining medical treatment or consultation if the visa was granted.
Evidence of funds to cover cost of treatment
Valid international passport
Visa application charge (for more than 3 months stay)
Chest x-ray reports (form 160) for patient and accompanying family members who are aged 11 and over. The panel doctor list can be retrieved from our website www.australia.org.my (For patients who are critically ill and cannot undergo a chest x-ray examination at the panel doctor's clinic, please contact the visa officer to discuss alternatives).
Medical examination (form 26) for children under 11 years of age
NOTE: Family members who are accompanying applicant to Australia are also required to complete form 48ME.
