PASSPORT APPLICATION FOR A MINOR
Checklist for New or Renewal of Passport for Minors (Under 18 years of age). Please read ALL instructions noted on the Application Form. The application form should be completed in BLOCK LETTERS using a black ballpoint pen.
Ensure you have completed the following sections before submitting your application:
► Section 11A & B - If the child is below 1 yr old, the Guarantor should be the Doctor who delivered your child.
If the child is above 1 yr old, you may complete either:
► Section 11A - Refers to a Guarantor who is the holder of an Australian Passport.
► Section 11B - Refers to a Guarantor who is a Professional
► Section 17 should be signed by a parent. If your child is 10 years or over they should sign their name on the signature label of Section 18 of the application form.
► Section 13 to 15. Consent from all parties with caring responsibility for the child, must be provided.
Please bring with you to the interview (see page 2 of the application form):
► Birth Certificate (Child’s)
► Previous Australian Passport
► Citizenship Certificate (if applicable)
► Parents’ Marriage Certificate
► Both parents current passports
► Evidence that at least one parent was an Australian Citizen or Permanent Resident at time of the child’s birth
► 2 Identical photographs (off-white background), 1 photograph endorsed by the Guarantor.
Please ensure your photographs conform to the requirements set out on page 4 of the passport application. Photographs that do not meet these strict requirements will not be accepted.
All documents in a foreign language must be accompanied by an English translation by an official translator.
** First time child application, the child (applicant) must be present at the time of interview.
Please note: The new policy, effective immediately, is that the dimensions of a child photo must be the same as adult photo dimensions - ie - chin to crown measurement be 32-36mm regardless of the age of the applicant.
Passport fee: RM 311 for 32 pages or RM 466 for 64 pages*.
Cash (Malaysian Ringgit) and Credit Card services available only at Australian High Commission, Kuala Lumpur. Personal cheques are not accepted.
Application lodged at Honorary Consul offices in Penang, Kuching and Kota Kinabalu, payment is accepted by bank drafts only. Bank drafts should be made payable to the "AUSTRALIAN HIGH COMMISSION"

The Passport counter is open from Mon-Fri 8.30am - 4.00pm except observed Public Holidays and no appointment is required.
• ONLY ORIGINAL DOCUMENTATION WILL BE ACCEPTED
• Providing all requirements (including contacting Guarantor and Witness) have been met the new passport will be ready in 10 working days (excluding observed Public Holidays) from the date the application is sent to Canberra.
SECTION 12 - GUARANTOR OF APPLICANT
Must fall into one of the following categories
- Possess a current Australian passport that was issued with at least 2 years validity
- Accountants – qualified Chartered Accountants. Members of (1A) Institute of Chartered Accountants in Australia, (1B) Australian Society of Certified Practising Accountants, (1C) National Institute of Accountants, (1D), Registered Tax Agents (Specify which below)
- Bank Managers except Managers of Bank Travel Centres
- Barristers, Solicitors and Patent Attorneys
- Elected representatives of Parliaments, Legislative Assembly of Norfolk Island and Municipal or Shire Councils
- Clerks of Courts
- Clerks of Petty Sessions
- Members currently serving in the regular Defence Force with at least five years continuous service
- Dentists
- Registered Medical Practitioners
- Chartered Professional Engineers
- Teachers - Full time who have been teaching for more than five years at schools or tertiary institutions
- Public Servants - Current full-time employees of Commonwealth, State, Territory or Local Governments or Statutory Authorities, who have been employed continuously for at least five years by their current employer
- Stipendiary Magistrates
- Registered Veterinary Surgeons
- Police Officers in charge of Police Stations, Police Sergeants and above with at least five years continuous service
- Judges
- Marriage Celebrants
- Optometrists
- Pharmacists
- Registered Nurses
